This site publishes evidence audits of public bodies and institutions. Each audit compiles documented failures, failings and controversies from primary sources — regulator judgements, Ombudsman decisions, coroner reports, court rulings and reputable press — and links every single claim to its source.
The project exists to make institutional accountability easy to check. Rather than assertion, every item carries a label: VERIFIED (primary source), DOCUMENTED (reputable press or official statistics), or UNVERIFIED (advocacy only, fenced).
The home page ranks the audits in a league table by negative findings — the number of adverse, sourced findings documented for each body, excluding each audit's own balance/context item.
New audits are added as research is completed, and each existing audit is re-scanned daily for new findings.
