Methodology
Every finding on this site is built from a source document. The process:
- Primary sourcing. We prefer regulator judgements (CQC, RSH, Ofsted), Ombudsman decisions (Housing/Local Government), coroner Regulation 28 / Prevention of Future Deaths reports, and court rulings.
- Labelling. Each item is tagged VERIFIED (primary source), DOCUMENTED (reputable press or official stats), or UNVERIFIED (advocacy only, fenced off).
- Balance. Where a body is currently rated adequately (e.g. CQC ‘Good’), that is stated honestly rather than omitted.
- No fabrication. We do not invent figures, quotes or URLs. If a search returns nothing solid, nothing is published.
- Daily re-scan. Each audit has an automated daily scraper that hunts for new verified findings and publishes them with the date.
The league ranking counts negative findings = total sourced findings minus the audit's single balance/context item. Higher means more adverse findings documented.
